Liability Waiver
Karate includes strenuous physical activity and contact. This waiver must be reviewed and accepted by an adult participant or a minor student’s parent or legal guardian.

Voluntary participation and risk
I understand that martial-arts training may involve falls, impact, physical contact, equipment, and strenuous exercise, with risks including bruises, sprains, fractures, concussion, or other injury. I voluntarily accept the ordinary risks of participation for myself or the minor student I represent.
Health and instructions
I will disclose relevant medical conditions, injuries, allergies, and limitations, seek medical advice when appropriate, and ensure the participant follows instructor directions, safety rules, and equipment requirements. The academy does not provide medical advice.
Emergency response
If I cannot be reached, I authorize reasonable first aid and emergency assistance for the participant. I understand that I remain responsible for resulting medical costs.
Release
To the extent permitted by Saskatchewan law, I release SHOTOKAN Karate Regina, its instructors, volunteers, and facility partners from claims arising from the ordinary inherent risks of voluntary participation, except where liability cannot legally be excluded.
Electronic acceptance
Typing the required electronic signature and submitting acceptance records the policy version, date and time, account, student, and an IP address where appropriate. Electronic acceptance has the same intent as signing a paper copy.
